Instructions: For each statement, please rate how much it applies to you using the following scale:

1 = Not at all/Rarely
2 = Sometimes/Occasionally
3 = Moderately/Sometimes
4 = A lot/Often
5 = All of the time/Always

Self-Related Mentalization

1. I find it important to understand reasons for my behavior.
2. When I get upset I am not sure whether I am sad, afraid, or angry.
3. I am often confused about my exact feelings.
4. I find it difficult to admit to myself that I am sad, hurt, or afraid.
5. I can easily describe what I feel.

Other-Related Mentalization

6. I can empathize with other people's feelings.
7. When someone annoys me I try to understand why I react in that way.
8. I can make good predictions of other people's behavior when I know their beliefs and feelings.
9. I find it difficult to understand someone's reasons before s/he tells me something.

Motivation to Mentalize

12. I find it important to understand what happens in my relationships with people close to me.
